Description

ABOUT THE CENTER FOR COMMUNITY CAPACITY DEVELOPMENT (CCCD)

IPHI's Center for Community Capacity Development (CCCD) works with community collaboratives, public health departments, health care organizations, state agencies, and other partners to build capacity for policy, systems, and institutional change. The CCCD provides training, consultation, and strategic planning services to public health system partners to support activities including community health assessment, action planning, strategic planning, performance management, quality improvement, and implementation and evaluation of evidence-based strategies, policies, and systems change.

Currently, the CCCD is leading implementation planning for the Illinois State Health Improvement Plan (SHIP) to advance health equity in 5 priority areas: chronic disease, emerging diseases, maternal and infant health, mental health and substance use, and racism as a public health crisis. This work involves supporting five action teams, policy and data subcommittees, and the SHIP Partnership, an appointed oversight body.

In addition, CCCD serves as the backbone organization for the Alliance for Health Equity, which is a collaboration across hospitals, government, and community partners working to improve health equity, wellness, and quality of life across Cook County. Since 2015, the Alliance has produced four collaborative community health needs assessments (CHNAs) and fostered partnerships and policy and systems change initiatives related to social and structural determinants of health and mental health and substance use disorders.

The Alliance supports several systems change initiatives to address priority community health needs, including a Food is Medicine Committee and the Illinois Medical Respite Capacity Building Initiative, which advances expansion of medical respite and recuperative care programs for people experiencing homelessness in over 20 communities across Illinois with funding from the state's Office to Prevent and End Homelessness (OPEH).

JOB SUMMARY

The Program Director is responsible for overseeing administrative, financial, and project management activities across the Center for Community Capacity Development (CCCD) in collaboration with the Center Directors, central administration, finance, and contracts teams. The duties of the Program Director include supporting the long-term strategic and programmatic goals of the center from an administrative perspective. This includes supporting financial management (budgeting and monitoring), financial sustainability (grant writing and reporting), coordination between program and contracts teams, responding to funder requests, report development, and engagement with community partners through collaboratives and other projects.

This role also includes supervising team members.

This position is full-time, exempt, and reports to the Center Director. IPHI's standard work week is 37.5 hours.?

The IPHI operates in a dynamic funding environment, and the availability of specific program funding may evolve over time. While the position you are applying for is currently supported by existing funding, future changes in funding sources could impact the associated program. IPHI is committed to supporting our staff and will work to transition employees into other programs within the organization should program funding shift.
