Rehabilitation Case Coordinator I – Illinois Department of Human Services (Chicago, IL)

Salary: $51,800.00 – $55,000.00

Position Overview

The Division of Rehabilitation Services (DRS) is the state's leading division dedicated to individuals with disabilities, focusing on diversity, equity, inclusion, accessibility, and racial justice. The mission of the Division is to assist those with disabilities in achieving employment, education, and independent living goals. This role coordinates customer case records and documents in a field office, performing routine clerical tasks essential to securing and maintaining sensitive case files.

The Coordinator facilitates the purchase and delivery of services, monitors customer case status, prepares annotated reports for counselors, and interacts with field office staff as well as internal and external customers via person, phone, and email. A self‑motivated, detail‑oriented individual with strong communication skills is sought.

Essential Functions
  • Works with counselor(s) and the case‑management system to determine the daily priority of customer service delivery.
  • Meets routinely with counselors to review case‑management activities and problems encountered.
  • Assists applicants, program participants, family members/guardians/caregivers, vendors, and referring agencies with understanding program rules, policies, eligibility criteria and various aspects of the vocational rehabilitation program.
  • Triages and prioritizes incoming mail, email, phone calls, voicemail, and drop‑in customers.
  • Meets with Vocational Rehabilitation Program staff and/or office supervisor to plan and discuss general casework activities.
  • Performs other duties as reasonably required, within the scope of the above duties.
