Job Summary:
Transitions at Home is seeking compassionate caregivers to assist clients within their homes. The majority of our clients are located in Washburn and Sawyer counties, but we also serve Burnett, Barron, Bayfield, and Rusk counties. Caregivers are reimbursed for mileage and travel time to and from clients' homes.

The ideal candidate will be dependable, compassionate, and willing to assist with a variety of tasks to help clients remain comfortable and safe in their homes.

This is a casual position, meaning work hours are not guaranteed and will vary based on client needs. We are seeking a caregiver who can be matched with clients based on availability.

Key Responsibilities:

Provide companionship and engage in conversation with clients

Help with meal preparation and light housekeeping

Assist with personal care, including bathing, dressing, and grooming

Change linens and do laundry

Offer medication reminders and ensure safety monitoring

Run errands such as shopping and transportation to doctor's appointments

Assist with exercises prescribed by physicians or physical therapists

Requirements:

Valid driver's license

Access to a reliable vehicle

Car insurance

Experience working as a Caregiver preferred

CNA certification NOT required—we are looking for dependable and compassionate individuals who can assist with the outlined tasks.
