Position Overview
Illinois Department of Public Health (IDPH) is seeking a Social Worker Surveyor I to conduct routine health and safety surveys of healthcare facilities. The Surveyor will travel to inspect compliance with state and federal requirements, interview patients, staff, and administrators, investigate abuse or neglect complaints, and prepare reports on deficiencies.

- Perform surveys of all provider types, including hospice, home health, hospitals, and ESRD facilities, to assess compliance with licensure and federal regulations.
- Investigate complaints concerning safety and regulatory compliance in healthcare facilities.
- Explain and outline deficiencies as part of the health facility survey team.
- Complete required surveyor training and maintain up‑to‑date knowledge of regulations.
- Participate in staff meetings and assist with orientation of new surveillance staff.
- Perform any additional duties related to survey responsibilities as assigned.
- Master’s degree in Social Work from an accredited institution.
- Validity of an Upward Mobility Program Credential.
- Experience with state and federal health care facility inspection regulations.
- Understanding of social work theory and the nature of professional practice.
- Ability to draft complex written and oral reports.
- Experience evaluating services provided by hospice, home health, hospitals, and ESRD facilities.
- Proficiency in data collection, analysis, and interpretation of technical materials.
- Proven ability to maintain professional relationships with diverse stakeholders.
- Strong critical thinking, judgment, discretion, and confidentiality during surveys.
- Skilled in Microsoft Office, including Word, Excel, PowerPoint, Outlook, and Smartsheet.
- Work requires travel and a valid driver’s license.
- Overtime work, including evenings and weekends, may be required.
- Position is subject to the revolving door restrictions under 5 ILCS 430/5‑45.
IDPH oversees community health and safety across Illinois. With a budget exceeding $2 billion and over 1,200 employees, IDPH maintains oversight of disease prevention, health promotion, and regulatory compliance.
