Daily Responsibilities:
- Coordinates care for participants and provides educational information regarding medication and disease states
- Performs assessments and data analysis of claims to determine appropriate care needed
- Develops/implements appropriate care plans based on assessment findings
- Identifies barriers and potential risks
- Provides needed social support and resources needed for care including benefits/community services
Hours for this Position:
- Monday-Friday 8am-5pm

Requirements:
- Must be Licensed Social Worker (LSW) or Licensed Clinical Social Worker (LCSW)
- 2+ years of professional experience as an LSW or LCSW
- 2+ years of Case Management, Community Health, or Care Coordination experience
- Must be able to travel locally (valid driver's license and auto insurance required)
