General Job Description
This position is responsible for providing direct clinical and case management services, including: intake and assessment with new tenants, collaborative development of individual service plans with tenants, crisis intervention, individual counseling and providing therapeutic group activities to help tenants meet their goals. The case manager provides linkages and coordinates resources/referrals. Case managers assist tenants in goal attainment, including but not limited to: maintaining their housing, obtaining and maintaining sustainable income, and achieving health and wellness goals.
Valid Illinois driver's license, proof of insurance, and must have own vehicle. Mileage reimbursement provided.
We offer supervised clinical hours towards LCSW and LSW certifications.
Job Responsibilities and Duties Case Management Services- Provide case management services including intakes, assessments, service planning, staffing, individual counseling, referral, and advocacy.
- Provide crisis intervention in mental health and/or medical emergencies.
- Assist with applications and advocacy related to public benefits
- Plan and implement therapeutic group activities.
- Work with tenants to achieve their goals related to housing, income and health.
- Regularly assess and respond to tenants' motivation and readiness using motivational interviewing skills
- Engage in clinical supervision, in both group and individual formats
- Work with tenants to address issues in order to prevent eviction
- Prepare and maintain current tenant files
- Enter all services and required information into the HMIS and other service and outcome tracking databases as required.
Position Qualifications and Requirements
Education:
Bachelor's degree in social work or related field required. Master's Degree preferred.
Job

2-3 years case management experience
Special Knowledge &
Skills:
Knowledge of issues surrounding homelessness, mental illness and substance use; effective oral and written communication skills; conflict management and crisis intervention skills; computer literacy
Unusual
Physical Requirements:
Ability to work occasional evenings, weekends and holidays; carries a pager and/or a cell phone to respond to crises.
