Position Summary

The Advanced Practice Registered Nurse - Certified Nurse Practitioner (APRN- CNP) or Certified Physician Assistant (PA-C) reflects the mission, vision, and values of University of Illinois Hospital & Health Sciences System, adheres to the UI Health Code of Ethics and Corporate Compliance Program, and complies with all relevant policies, procedures, guidelines, and other regulatory and accreditation standards. The ideal candidate holds a degree from an accredited Nurse Practitioner or Physician Assistant school or college and has Board Certification from Nationally Accredited Certifying body, based on the APP category.

The APP functions as part of the multidisciplinary health team in accordance with privileges approved by the credentialing committee to provide high-quality, cost-effective advanced care to patients.

Duties & Responsibilities

* Works within the Bylaws of the University of Illinois Hospital & Health Sciences System and in adherence to hospital and divisional policy.

* Works collaboratively with other providers in the care of transplant recipients.

* Performs an accurate history and physical examination of patients. Conducts comprehensive or episodic health assessments and develops patient care plans.

* Performs procedures according to clinical delineation of privileges.

* Provides prevention strategies and management of acute and chronic health problems.

* Orders, coordinates care, interprets tests, treatments, and therapies within the scope of practice for pre or post-transplant recipients.

* Educates patients and caregivers on pre and post-transplant patient care.

* Ensures that all transplant patient coordination and patient education is documented and follows transplant regulatory requirements and within UI Health policies/procedures.

* Orders all testing for evaluation as needed and escalates results to collaborating physician as necessary.

* Prescribes medications, diet, supplies, prosthetic devices, and other home health aides or medication according to clinical privileges. Provides clear, concise, accurate, and legible documentation regarding patient care in the medical record.

* Advocates for patients, families, co-workers, the community, and the nursing profession.

* Integrates evidence-based knowledge and clinical expertise in decision-making.

* Participates in peer review and competency assessment activities.
