The Registered Dietician develops, implements and oversees a plan of care for patients based upon an assessment of nutritional needs.
Provides nutritional education, counseling and interventions in the management of nutritional status and disease management and in accordance with current Federal, State and facility standards, guidelines and regulations.
- Performs patient assessments and consultations.
- Conducts rounding to drive patient satisfaction.
- Conducts comprehensive nutritional assessments that consider the patient's history, lifestyle, and biochemistry.
- Collaborates with patients to understand their health goals, challenges, and dietary preferences.
- Assesses root causes of health issues and designs individualized nutrition plans.
- Develops and implements personalized nutrition plans that incorporate whole foods, supplements, and lifestyle changes to support optimal health.
- Educates patients on the relationship between nutrition, lifestyle, and overall health, emphasizing the role of diet in preventing and managing chronic conditions.
- Monitors and adjusts treatment plans based on patient progress and evolving needs.
- Utilizes assessment and evaluation techniques that consider the varied needs of age‑specific populations as well as cultural, religious, and ethnic concerns
- Actively manages patients across their care plan to achieve the desired health outcomes.
- Communicates complex nutritional information in a clear and supportive manner to patients and their families.
- Stays informed on the latest research and trends in nutrition.
- Bachelor's degree from a 4‑year college or university.
- Registered Dietitian with the Commission of Dietetic Registration and Licensed Dietitian status.
- Previous experience as a Clinical Dietitian, preferably with high‑risk senior populations in long‑term care settings, is preferred but not required.
