Lighthouse Foundation of Chicagoland is seeking a Part-Time Community Health Worker in Chicago to provide client-centered breast and chest health education, screening navigation, care coordination, and outreach. Work is hybrid with 10–14 clients per quarter and collaboration with a Breast Cancer Peer Advocate.
You will support access to preventive screenings, navigate referrals, and maintain confidential documentation while engaging with Black LGBTQ+ communities across the South Side.
