The Community Care team is multidisciplinary, including RN Community Care nurses, LPN Community Care nurses, Community Social Workers, and Community Health Coordinators. They work with our highest complexity patients and their primary care physicians to meet their medical and social needs, fully engage them in our intensive primary care model, and maximize their healthy time at home.

As the RN Community Care Nurse, you will serve as a clinical lead, coordinate team efforts to stabilize highest risk patients, supervise other team members, prioritize team efforts, and provide clinical supervision to ensure they engage in care.

• Provide in‑home and telephonic visits to high‑risk patients to prevent hospital admission or readmission.
• Conduct initial assessments, develop care plans, and supervise LPN follow‑up visits and discharge documentation.
• Supervise LPN and patient visits, provide education, and oversee appropriate discharge from case management.
• Perform clinical and social determination of health screenings, disease monitoring, medication monitoring, health education, and self‑care instructions in the outpatient in‑home setting.

• Oversee LPN with clear plan of care and mandatory education during all LPN visits.
• Conduct initial case management assessment to determine outpatient needs and ensure plan reflects patient needs and available community services.
• Complete individual plan of care with patients, family/caregiver, and care team members; communicate instructions and methodology to ensure correct implementation.
• Assess safety and security of care environment, caregiver capacity, educational needs, and coordinate, report, document, and follow‑up on multidisciplinary team meetings.
• Assist patients with access to community/financial resources, refer cases to social worker as appropriate, and perform urgent home visits under the direction of the patient’s primary care physician.
• Perform other duties as assigned and modified at manager’s discretion.

$35.8 – $51.17 Hourly

• Registered Nurse (RN) with experience in primary or community care.
