Family Nurse Practitioner

Family Nurse Practitioner (FNP)

Family Medical Doctor / Medical Director

Delivers high-quality primary and preventive care for adults in an integrated community‑mental‑health setting, focusing on chronic‑disease management, wellness promotion, and collaboration with behavioral‑health teams.

• Conduct full health assessments, diagnose and manage acute and chronic conditions.
• Provide medication management, patient and family education, and minor procedures.
• Order and interpret diagnostic testing.
• Collaborate with physicians, therapists, case managers and mentor nursing staff.
• Maintain thorough EHR documentation and participate in quality improvement initiatives.

• MSN or DNP, national NP board certification (AANP/ANCC).
• Active IL NP license;
  Controlled Substance/DEA and Full Practice Authority preferred.
• At least 12 years of primary‑care NP experience; behavioral health exposure valued.
• Proficiency with EHR (athena
  
  One) and strong teamwork and communication skills.
• Can work in an ambulatory clinic environment with some standing/walking; must meet standard clinical credentialing and vaccination requirements.
