Seeking a full‑time RN Case Manager. Responsibilities include performing post‑discharge calls, case management evaluations, and acquiring services for discharge planning.

• Perform post‑discharge calls to ensure patients are doing well after discharge and answer any questions or concerns regarding patients’ health.
• Assess new patients by gathering information, reviewing diagnoses, and analyzing medical test results.
• Develop complete plans of care to address the needs of patients, including, but not limited to, assessing patients at risk of adverse events and re‑admission.
• Educate patients about their care plan and health care options, helping them make key decisions about their health and post‑discharge options.
• Schedule appointments for doctors’ visits, therapies, and other medical services.
• Revise care plans based on patients’ progress and input from physicians and other medical providers.
• Update patient medical records after each contact.
• Offer emotional support for patients and their families.

• Registered Nurse.
• Available to work part‑time.
• Available to work some weekends.
• Experience with case management (preferred).
