About the Role
We are seeking a Family Nurse Practitioner (FNP) to deliver comprehensive, patient-centered primary care to individuals and families across the lifespan. The ideal candidate is a collaborative clinician who practices evidence-based care, communicates clearly, and thrives in a team-oriented environment focused on quality outcomes and an excellent patient experience.
Key Responsibilities- Provide primary care services to patients of all ages (pediatrics through geriatrics), consistent with FNP certification and clinic scope
- Perform comprehensive assessments, histories, and physical exams
- Diagnose and manage acute and chronic conditions (e.g., diabetes, hypertension, asthma/COPD, depression/anxiety)
- Deliver preventive care including wellness visits, screenings, immunizations, and lifestyle counseling
- Order and interpret diagnostic tests (labs, imaging, EKGs) and coordinate follow-up care
- Prescribe medications and develop treatment plans in accordance with applicable state laws and organizational protocols
- Educate patients and families on diagnoses, medication use, and care plans to support adherence and self-management
- Coordinate referrals and collaborate with specialists and community resources when appropriate
- Document accurately and timely in the electronic health record (EHR) and comply with quality and regulatory standards
- Participate in team huddles, care coordination, and quality improvement initiatives
- Graduate of an accredited Nurse Practitioner program (MSN or DNP)
- Current Family Nurse Practitioner (FNP) national certification (e.g., AANP or ANCC)
- Active (or eligible) state licensure as an APRN/NP
- Current BLS certification (ACLS preferred based on setting)
- DEA registration (or ability to obtain), as applicable
- Strong clinical judgment, patient communication skills, and ability to work in a team-based care model
- 1+ years of experience in family medicine, primary care, urgent care, or community health (new grads may be considered depending on support structure)
- Experience with chronic care management and population health / quality metrics
- Comfort with telehealth and/or hybrid care models
- Experience performing common office procedures (based on privileges), such as suturing, joint injections, I&D, and skin lesion evaluation
- Outpatient primary care setting (typical weekday schedule; some clinics may offer extended hours)
- Supported by clinical staff (MAs, nurses), care coordination, and provider leadership
- Patient panel and visit types vary (preventive care, chronic disease follow-ups, same-day visits)
