Responsibilities
  • Precise Daily Documentation: Meticulously and accurately document daily patient care, detailed observations, and comprehensive responses to treatments, ensuring all medical records are thorough, up-to-date, legally compliant, and reflective of the highest clinical standards.

  • Accurate Medication Order Transcription: Transcribe physicians' orders to MARs (Medication Administration Records), ensuring proper, safe, and timely medication delivery to every resident.

  • Seamless Pharmacy Coordination: Notify the pharmacy of new orders and changes to existing prescriptions to facilitate continuous, uninterrupted medication availability.

  • Safe Medication Administration: Prepare and administer medications as ordered, adhering to established protocols, safety guidelines, and regulatory requirements.

  • Collaborative Interdisciplinary Engagement: Participate in the interdisciplinary care team to contribute to care plans that address each resident's needs.

  • Vigilant Patient Monitoring and Assessment: Monitor patient conditions, identify changes or concerns, and implement timely nursing interventions to optimize outcomes.

  • Comprehensive Wound Care Management: Provide wound care as prescribed, assess status, apply dressings, and document healing progress.

  • Effective Communication and

    Education:

    Serve as a primary point of contact for residents and families, communicating care plans and health status, and providing education on health topics.

  • Adherence to Clinical Policies: Adhere to all facility policies, procedures, and safety protocols to maintain a secure and efficient environment.

  • Dedicated Support: Perform other duties as assigned by the Director of Nursing or Administrator, contributing to overall success and quality of patient care.

Qualifications
  • An active and unrestricted Registered Nurse (RN) License within the state of Illinois.

  • A commitment to patient-centered care and compassionate approach in all interactions.

  • Excellent communication and interpersonal skills; ability to collaborate within a diverse, multidisciplinary team.

  • Ability to prioritize tasks, manage time, and remain calm in a dynamic healthcare environment.

  • Basic computer proficiency for electronic health records and communication.
