Geriatrics Nurse Practitioner

Seeking experienced Nurse Practitioners to provide effective and equitable value-based primary care to Medicare patients in the Chicago, IL area.

• Full-time, Mon - Fri, 8am-5pm
• 13-16 patients per day
• Geriatric population
• Multiple locations within the Chicago area to choose from
• Spanish Speaking required for some locations

• Master of Science in Nursing, Doctor of Nursing Practice, or NP Certificate Program graduate required
• National certification as a Family, Adult or Adult-Gerontology Primary Care Nurse Practitioner through AANP or ANCC
• Experience in primary care :
  Internal medicine, geriatrics, or family medicine preferred
• Understands HCC (Hierarchical Condition Categories) documentation, ICD-10 (International Classification of Diseases-10) Coding, and Health Risk Assessments (HRAs).

• Competitive Salary - $105,000-$148,500 Wider range depending on experience
• Quarterly bonus based on quality metrics
