Registered Nurse - Geriatric

Ryz Labs is hiring a registered nurse to provide services to patients/clients in their places of residence.

• Utilize the nursing process to plan, implement, and evaluate patient/client care and/or services provided to client(s) receiving contracted services.
• Initiate the plan of care when directed and request any needed revisions from the prescriber as the patient's status and needs change.
• Incorporate therapeutic, preventive, and rehabilitative nursing actions based on patient diagnosis and established goals.
• Intervene as applicable in response to changes in patient/client condition and report such changes to the appropriate physician or supervisor.
• Administer medications and treatments as prescribed, and monitor and document the effectiveness of the intervention.
• Review plan of care and program protocols, identifying areas for healthcare teaching/education, care coordination, goal setting, and discharge planning needs.

• Current Registered Nurse (RN) License for the state in which the nurse practices.
• Current Health Certificate (per facility Registered Nurse contract or state regulation).
• Current PPD or Chest X-Ray.
• Current BLS card.
• Experience working with elderly people.
• One year prior Registered Nurse (RN) experience preferred.
