PAY: $60.00 - $100.00 per visit PRN rates, mileage reimbursement, iPads with company-paid cellular data, opportunities for full-time positions with benefits.
BRIEF JOB DESCRIPTION: The registered nurse plans, organizes, and directs home care services emphasizing health education/experience. The RN Case Manager is responsible for traveling to a patient’s home to administer the services dictated by their physician and At Home Quality Care while helping patients maintain their independence and quality of life.
Essential Job Functions / Responsibilities- Assess and chart observations of the patient’s condition and needs at each home visit.
- Complete evaluation tasks, including medication reviews and vital signs.
- Initiate the Plan of Care and make necessary revisions as patient status and needs change.
- Provide education to patients and family members on proper home health procedures and strategies.
- Develop a care plan with goals based on nursing diagnosis and incorporate therapeutic, preventive and rehabilitative nursing actions.
- Administer medications and treatments as prescribed by the physician.
- Provide health care instructions to the patient as appropriate per assessment and plan of care.
- Identify discharge planning needs as part of the care plan and prepare patient and family for discharge.
- Act as Case Manager when assigned by Clinical Supervisor and coordinate patient care for the assigned caseload.
- Coordinate with physicians, physical therapists, occupational therapists, MSWs and other professionals involved in the patient care plan.
- Graduate of an accredited school of nursing.
- Current licensure in the state of Illinois and current CPR certification.
- Management experience not required. Responsible for supervising home health aides.
- Excellent observation, verbal and written communication skills, problem-solving skills, basic math skills, nursing skills per competency checklist.
- Prolonged walking or standing; able to lift, position, or transfer patients; able to lift supplies and equipment; considerable reaching, stooping, bending, kneeling, or crouching. Visual acuity and hearing to perform required nursing skills.
- Must be a licensed driver with an automobile insured under the state of IL; automobile in good working order.
