What does a Transitions at Home RN Case Manager do?
Transitions At Home Registered Nurse Case Managers provide skilled nursing care to include assessments, medications, treatments, and coordinates services, counsels the patient and family/significant others in their place of residence in accordance with the patient’s physician orders and plan of care under the direction and supervision of the RN Clinical Manager.
Hours:
- 40 hours per week 8:00 am - 4:30pm
- Monday through Friday
- No Night Shifts (also known as PM and NOC Shifts)
- Limited on-call rotation
- $34- $37 per hour starting wage, depending on experience

Qualifications /
Job Requirements:
- Active Wisconsin Registered Nurse License
- Two (2) years’ nursing experience, 1 year in Home Health (preferred)
- CPR Certification
- A valid Wisconsin Driver's License and Car Insurance
