* Summary:

** The Community Health Nursing Manager manages the day-to-day operational activities and responsibilities of the community health nursing team. They collaborate with providers and all members of the care team to support high quality, well-coordinated patient care. They directly supervise all community health center nurses; and ensure safe medication and infection control practices across locations.

* Essential Duties & Responsibilities:

*** Hire, train, supervise, support and evaluate community health nurses.
* Directly manage community health nurses– assign and direct work, conduct performance reviews, address complaints, and monitor productivity.
* Establish staff competency in nursing practice to assure patient safety, patient satisfaction and quality; ensure staff compliance with organization policies.
* Review, approve and manage community health nurses schedules and assures appropriate coverage for key nursing tasks including triage. Use electronic timekeeping system to review and approve requests for time off, approve timecards and communicate staffing issues to health center leadership.
* Assure timely and effective communication to community health nurses regarding changes to policies, procedures, and workflow.
* Coordinate with the Nursing, Risk, Compliance & Safety VP and Associate Operations VP to assure a safe patient care environment, including managing infection control practices, and handling the supply and storage of health center medications, clinical supplies and equipment.
* Establish standard practice and procedures for nursing role in transitions in care and referral coordination, including accessing necessary values and reports, providing information and support to patients and families, and working with the and provider leadership to assure effective workflows and appropriate follow up.
* Contribute to quality assurance, patient care policies and other organizational activities.
* Contribute to organizational goals and outcomes.
* Maintain a nurse practice at 10-15 hours per week.
* Other duties as assigned.
