Lawndale Christian Health Center is hiring a shelter-based care manager. We provide onsite, primary care at 40 different shelters throughout Chicago. The Shelter-based care manager will support this important work by:

• Comprehensively addressing our patient's vast social needs through insurance and benefit navigation, among other resources.
• Collaborating with shelter staff and housing partners to support our patient's housing goals.
• Coordinating additional healthcare services for our patients, including vision, dental or specialty care.

• Demonstrates excellent communication skills and is able to communicate clearly to patients, staff, and community agencies
• Strong ability to multi-task and work well in a fast-paced, team environment
• Demonstrated problem-solving skills
• Demonstrated effective interpersonal relationship and customer service skills
• Strong organizational and time management skills
• Embraces and personifies the mission of Lawndale Christian Health Center, by performing work in a whole person way, providing for the physical, emotional and spiritual needs of staff members and patients

• Bilingual in English and Spanish preferred
• B.S/B.A in social/health sciences required
• Two or more years of relevant experience preferred
