About the Role
We’re looking for a compassionate problem-solver who thrives in a mission-driven, team-oriented environment.

The Case Manager serves as the central hub of client care at The Ark. This role helps individuals and families navigate complex challenges with empathy and skill – developing collaborative care plans, coordinating internal and external supports, and helping clients move toward self-reliance and stability.

What You’ll Do

  • Serve as the “hub” of client support, coordinating care across social services, medical, housing, and financial programs.
  • Partner with clients to create individualized care plans aligned with their needs, strengths, and goals.
  • Provide motivational interviewing, active listening, and basic crisis intervention.
  • Assist clients with public benefits applications (SNAP, Medicaid, LIHEAP, SSI, etc.).
  • Maintain accurate, timely documentation and ensure continuity of care.
  • Participate in team meetings, trainings, and ongoing professional development.

What You’ll Bring

  • Education: Bachelor’s degree in Social Work (BSW) or related human services field required; MSW preferred
  • Experience: 2+ years in case management or social services
  • Skills:
  • Strong communication, rapport-building, and organizational abilities
  • Familiarity with motivational interviewing, trauma-informed care, or crisis intervention is a plus
  • Knowledge of community resources and referral systems
  • Understanding of basic mental health symptoms for screening and referral
  • Cultural humility and appreciation for Jewish values and traditions
  • Proficiency in Microsoft Word and Teams
