Case Manager – Home Study & Post Release Services (HS/PRS)

Salary: $64,500 annually.
Our Home Study and Post Release Services (HS/PRS) program supports unaccompanied children (UC) aged 0‑17 ½ from the Office of Refugee Resettlement (ORR) to families or sponsors. As a Case Manager you will conduct home studies, provide post‑release services, advocate for children and families, and ensure ongoing safety and well‑being.
Responsibilities- Conduct home studies and assess safety of the home and suitability of the sponsor.
- Provide post‑release services to child and sponsor, connecting them to community resources.
- Advocate for children and families, navigate systems, and coordinate medical, educational, and mental health services.
- Monitor placements on an ongoing basis, ensuring safety and well‑being.
- Manage a high‑turnover caseload of up to 40 youth, ensuring each youth is enrolled and attending school.
- Respond to client emergencies inside and outside work hours.
- Travel throughout the region, sometimes up to 3 hours by personal vehicle.
- Maintain all assessments, reports, and case notes in hardcopy and electronic files.
- Ensure coordination of initial intake, safety screenings, mental‑health assessments, and services schedule.
- Contact child and sponsor within 24 hours of assignment.
- Bachelor’s degree in a relevant behavioral, social science, or related field.
- Experience conducting home studies per state requirements.
- Strong professional writing skills for assessment reports.
- Knowledge of local community social services.
- Experience working with immigrant populations.
- One to three years’ progressive case‑management experience (preferred).
- Child‑welfare experience and professional licensure strongly preferred.
- Proficient in Microsoft Office.
- Bilingual – English/Spanish required.
- Valid driver’s license in the region and reliable personal vehicle.
- Ability to travel extensively, both ground and air.
- Strong interpersonal skills, able to work with diverse cultures and non‑English speakers.
- Computer work requiring long periods of sitting at a desk.
- Ability to lift up to 30 pounds.
- Time driving or using public transportation for client services.
- Regular hours Monday‑Friday 9 a.m.–5 p.m., with flexibility for program needs.
- Required availability for afternoons, evenings, and weekends.
- 120 hours vacation; 12 sick days per year.
