Hesed House in Illinois is seeking a LIGHT-House Case Manager to support our Comprehensive Resource Center, serving the homeless and vulnerable with case management, counseling, and connections to services.
You will establish goals with participants, document progress, coordinate housing, medical, legal, and job coaching resources, and ensure safety within the unit. This full-time exempt role requires weekend availability and a commitment to Harm Reduction and Housing First principles.
