Description:

The Care Coordinator is responsible for coordinating the care and services for assigned patients during episodes of illness or treatment.

Collaboration with the patient, family, physician, patient care team, and payors is essential.

The role involves using advanced clinical skills to facilitate the provision of care.

Key responsibilities include assessment, planning, intervention, and evaluation principles of the nursing process.

The Care Coordinator promotes and evaluates the effective utilization of resources.

Current clinical knowledge and awareness of community services are utilized in this role.

Assuming a leadership role with the patient care team is required to achieve optimal clinical and resource outcomes.

The evaluation and analysis of quality/utilization cases, admission/discharge criteria, and readmission rates is part of the role.

Participation in ongoing outlier reviews is  collaboration with the patient and family, the Care Coordinator works with a multidisciplinary team to ensure efficient and cost-effective care.

Availability to the patient and family as a resource for communication among providers is maintained.

The Care Coordinator remains involved until the patient is appropriately discharged or transferred.

Initiating appropriate clinical pathways based on admission diagnoses is part of the responsibilities.

Serving as a resource person for utilization management purposes is also required.
