Registered Nurse Field Case manager

As an RN Case Manager, you will work at the top of your licensure, working one‑on‑one with clients to provide customized care. Under the guidance of a supervising physician, you will develop care plans and apply nursing theories, skills and techniques to deliver quality care daily.

• Assess all medicines to identify ineffective drug therapy, adverse reactions, side effect risks, drug allergies and contraindicated medications.
• Establish realistic, measurable, observable goals consistent with the patient's diagnosis.
• Comply with applicable legal requirements, standards, policies and procedures, including code of conduct, HIPAA, and documentation standards.
• Demonstrate professional conduct, service excellence and commitment to Accent Care’s values and mission.

• Graduate from an approved nursing school and currently be licensed to practice as a registered nurse in the state of agency operation.
• Have at least one (1) year experience as an RN.
• Maintain valid CPR certification while employed in a clinical role.
• Possess sufficient endurance for tasks over long work hours.
• Be a licensed driver with a well‑maintained, insured automobile in good working order and able to travel to all business locations.
