About the RoleThe Quality RN supports our Quality Assurance and Performance Improvement (QAPI) program by reviewing clinical documentation, monitoring quality outcomes, identifying trends and risks, and collaborating with clinical leadership to improve patient care across our Home Health and Hospice programs. This position serves as a clinical quality resource and strategic partner to leadership, helping ensure that documentation supports the patient’s condition, plan of care, services provided, regulatory requirements, and organizational standards.
Rather than simply identifying problems, you’ll help find solutions, strengthen clinical practice, and turn quality findings into meaningful improvements.
- Perform clinical record reviews and audits for Home Health and Hospice patients
- Evaluate documentation for completeness, accuracy, consistency, and regulatory compliance
- Review clinical records for alignment between the patient’s condition, assessment findings, plan of care, interventions, and outcomes
- Identify documentation gaps, clinical risks, and opportunities for improvement
- Support accurate and timely completion of required clinical documentation
- Provide clear, constructive feedback to clinicians and clinical leaders
- Hospitalizations
- Emergency Department utilization
- Falls
- Infection trends
- Wound outcomes
- Medication management
- OASIS quality measures
- Home Health Value-Based Purchasing (HHVBP) metrics
- Home Health Star Ratings
- Hospice quality measures, including HOPE and CAHPS

- Live Discharges
- Patient complaints and grievances
- Adverse events and clinical incidents
- Other clinical and operational quality indicators
- Identify trends and opportunities for clinical improvement
- Collaborate with clinical managers to investigate quality concerns and determine root causes
- Help develop and implement Performance Improvement projects
- Establish baseline performance and monitor progress over time
- Assist with corrective action plans and follow-up monitoring
- Provide education and coaching based on audit findings and identified trends
- Help translate regulatory requirements and quality findings into practical improvements for frontline clinicians
- Support compliance with Medicare Conditions of Participation and other applicable federal and state requirements
- Assist with survey readiness and ongoing regulatory compliance
- Participate in preparation for state, federal, accreditation, and payer audits or reviews
- Support clinical record requests, ADRs, audits, and other quality-related reviews as needed
