Registered Nurse; RN

Schedule: Full-Time, Monday-Friday dayshift - 40 hours per week

Compensation: $85,000-$95,000 + $2,000 sign on bonus

The Columbus Organization is seeking a Registered Nurse (RN) to support individuals with intellectual and developmental disabilities (IDD) throughout Northern California (mainly Chico and Redding area). Most work will be done from facilities in Chico or Redding but occasional community based travel may be required. This nursing role combines clinical assessment, care coordination, health advocacy, and consultation to help ensure individuals receive appropriate medical services, supports, and resources to maximize their health and quality of life.

The RN will partner closely with healthcare providers, families, care facilities, and interdisciplinary teams to evaluate healthcare needs, support access to specialized services, monitor medical status, and provide expert clinical guidance. • Conduct comprehensive nursing assessments evaluating medical, environmental, psychosocial, educational, and self-care needs.
• Review and monitor the medical and nursing status of individuals receiving community-based services.
• Occasional work on mortality review committee
• Identify needs for specialized medical services, referrals, consultations, and healthcare resources.
• Collaborate with physicians, healthcare providers, families, and interdisciplinary teams to support individualized care planning and positive health outcomes.
• Serve as a liaison with hospitals, skilled nursing facilities, healthcare providers, and community agencies involved in care delivery.
• Participate in discharge planning and care coordination activities to ensure continuity of care.
• Coordinate specialty consultations and clinical services as needed.
• Develop nursing care plans, clinical recommendations, health protocols, and written reports.
• Provide education and training to caregivers, providers, and community partners regarding healthcare needs and best practices.
• Assist with healthcare program eligibility reviews, documentation, and regulatory requirements.
• Advocate for appropriate healthcare services and supports to ensure individuals receive quality care.
• Maintain accurate clinical documentation, case records, and reports.

• Active and unrestricted Registered Nurse (RN) license in the State of California.
• Bachelor's Degree in Nursing (BSN) preferred.
• Minimum of three (3) years of professional nursing experience.
• Strong clinical assessment, care coordination, and medical documentation skills.
• Excellent critical thinking and problem-solving abilities.
• Ability to work independently while collaborating effectively with interdisciplinary teams.
• Exceptional communication and relationship-building skills.
• Valid California Driver's License and reliable transportation.
• Willingness to travel throughout the assigned service area.
• Experience working with individuals with intellectual and developmental disabilities, public health, case management, care coordination, or community-based healthcare preferred.
• California Public Health Nurse certification preferred.
