Position Overview

Wellness Nurse – Council on Aging Department Employment Status:
Non-Exempt; UFCW 1459 Admin;
Full-Time (14 Hours/Week) Pay Grade:
Grade A11:
Step 01 - Step 04 Salary Range: $28.73/Hour - $37.61/Hour

Required:

Masters Degree in Nursing (or four or more years of related experience), Current license to practice as Registered Professional Nurse in Commonwealth of Massachusetts.

SUMMARY The City of Chicopee’s Council on Aging Department is hiring a Part‑Time Wellness Nurse who will provide one‑on‑one consultations with older adults and/or their families and/or their physician/pharmacist. The Nurse also provides the same services to frail homebound older adults and is responsible for all record keeping and follow‑ups.

Essential Duties
  • Provides initial assessments to Chicopee residents who are sixty (60) years or older.
  • Monitors and makes referrals, as necessary.
  • Counsels clients and/or family regarding health care and problems.
  • Tests blood sugar levels, high blood pressure, anemia, weight loss or gain, and/or depression, as necessary.
  • Informs older adults on proper medication management and medication information.
  • Provides instruction on the use of medical equipment.
  • Gives Vitamin B‑12 shots as prescribed by a physician.
  • Performs dressing changes and assesses for unhealthy use of alcohol and/or prescription drugs.
  • Provides nutrition counseling based upon evidence‑based programs.
  • Keeps accurate health records on each client.
  • Orders all supplies necessary for operation of the Wellness Center.
  • Performs physical exams.
  • Informs the Social Services Coordinator and/or other staff as appropriate of any changes in the elder’s well‑being or follow‑up that might be necessary.
  • Advocates or informs physicians and/or family and/or other health services of any follow‑up care the client might need.
  • Facilitates various support groups, including a support group for diabetics.
  • Plans health promotional programs.
  • Provides in‑home nursing services for those who have no alternative.
Qualifications
  • Registered Nurse with a Master’s Degree (M.A) – Required.
  • Four or more years related experience and/or training of direct care nursing elders; or an equivalent combination of education and experience – Required.
  • Current license to practice as a Registered Professional Nurse in Commonwealth of Massachusetts – Required.
