Position Details
This is a full‑time position based at the Chicopee, MA branch office.
Job DescriptionClinical Team Manager responsible for overseeing all patient care services, including staffing, care coordination, and plan of care development and updates within the home health setting.

- Provide oversight of all patient care services and personnel, including assignments and coordination of care.
- Plan, direct, and participate in the delivery of home health services to provide diagnosis‑specific patient care, coordinating services with providers, vendors, or facilities.
- Ensure timely start of patient care within recommended regulatory guidelines.
- Process and coordinate physician orders, staff assignments, and communications among the direct care team, patients, families, and physicians.
- Review and approve plans of care and evaluate proposed changes for clinical appropriateness.
- Integrate orders from all relevant physicians, ensuring they are approved by the responsible physician.
- Conduct regular OASIS reviews for all patients.
- Meet regularly with direct care team members to provide guidance and coaching.
- Initiate hiring requests, interview candidates, and recommend hiring of direct care staff.
- Communicate frequently with the Branch Director or Administrator on key performance metrics.
- Remain available for field work as needed.
- Perform other duties/projects as assigned.
- Work in a physically demanding, high‑stress environment.
- Current, unrestricted RN license valid in the state of work.
- Associate degree in a nursing related field.
- 4–5+ years of home care experience as a registered nurse in a home health or hospice environment.
- 1+ year of supervisory experience preferred.
- Proficiency with enterprise applications such as Homecare Homebase, Workday, and Microsoft Office.
- Willingness and ability to travel within the branch/office coverage area for field staff support and training.
- Must be able to travel 50% of time.
- Ability to sit, stand, bend, lift and move intermittently and lift 50–100 lbs.
- Valid driver’s license and reliable transportation.
The base salary for this position is $105,000 to $115,000 annually, based on education, experience, skills, and internal equity.
