The On-Call Registered Nurse provides after-hours clinical support to hospice patients and families to ensure continuous access to care, symptom management, and crisis intervention. This role supports the hospice’s obligation to provide 24-hour nursing availability as required by regulation.
Essential Duties + Responsibilities- Provide telephonic triage for patients, caregivers, and facilities
- Conduct urgent visits for symptom management, death pronouncement, and crisis intervention
- Implement standing orders and physician directives
- Perform comprehensive assessments during urgent visits
- Manage pain, respiratory distress, agitation, nausea, and other end-of-life symptoms
- Adjust care plan interventions in collaboration with physician
- Communicate after-hours events to primary case managers and IDG
- Coordinate pharmacy, DME, and emergency services when necessary
- Facilitate transitions in level of care
- Provide education regarding symptom expectations and medication use
- Offer emotional support and anticipatory guidance
- Support families during active dying phase
- Complete timely, accurate documentation supporting interventions and level of care
- Document triage calls, visits, medication changes, and physician communication
- Ensure handoff notes are completed for next business day
- Follow response time standards and escalation protocols
- Adhere to medication management and controlled substance procedures
- Complete incident reports when required
- Follow infection control and safety guidelines
- Maintain availability during scheduled on-call shifts
- Participate in rotation scheduling and coverage planning
- Ensure readiness of supplies and equipment for after-hours visits
- Demonstrate understanding of 24/7 coverage requirement
- Participate in chart audits and education related to documentation
- Contribute to QAPI initiatives related to after-hours utilization
- Graduate of accredited nursing program
- Active RN license required in the state of practice
- Hospice experience strongly preferred
- Experience with triage and crisis care
