What You'll Do:
- Manage an assigned caseload of home health patients
- Complete comprehensive assessments and ongoing evaluations
- Develop, implement, and update individualized plans of care
- Coordinate services with physicians, therapists, caregivers, and clinical leadership
- Educate patients and families on medications, treatments, disease management, and safety
- Monitor changes in each patient's condition and communicate significant findings
- Maintain accurate, timely clinical documentation
- Help patients achieve better outcomes while remaining safely at home
What You'll Need:
- Current, unrestricted RN license
- At least one year of clinical nursing experience
- Associate degree in Nursing required; bachelor's degree preferred
- Previous home health or hospice experience preferred
- Homecare Homebase experience preferred
- Dependable vehicle, valid driver's license, and current auto insurance
- Ability and willingness to travel throughout the assigned territory
- Sit, stand, bend, lift and move intermittently and be able to lift 50–100 lbs.
