Position: Hospice RN (Registered Nurse)

Hospice RN (Registered Nurse) in Chicopee, Massachusetts

We are seeking an experienced and compassionate Hospice RN to join our dedicated team at Harmony Hospice. As a Hospice RN, you will play a vital role in providing high-quality, compassionate care to our patients and their families.

Job Summary:

As a Hospice RN, you will be responsible for planning, coordinating, and providing hospice care in accordance with the patient's plan of care. You will work closely with our interdisciplinary team to ensure that our patients receive comprehensive and personalized care, addressing their physical, emotional, and spiritual needs.

Responsibilities:
  • Coordinate hospice care by working with other team members, patients, and family members to ensure needs are met
  • Evaluate the effectiveness of hospice services to the patient and family on an ongoing basis
  • Utilize the nursing process to provide professional nursing care
  • Consult with attending physicians and Hospice Medical Directors concerning changes in patients' conditions
  • Coordinate patient services with other members of the IDG and contracted services, when needed
  • Submit complete, accurate, and relevant clinical notes in a timely manner
  • Participate in IDG and case conferences
  • Develop and implement the initial plan of care and work with other members of the IDG in completing and maintaining the comprehensive plan of care
Qualifications:
  • Graduate from an accredited School of Nursing
  • Licensed in the State of Massachusetts
  • One year of RN experience;
    Hospice experience preferred
  • Excellent assessment skills and problem-solving skills
  • Strong organizational and time management skills
  • Ability to communicate well, both verbally and in writing
  • Reliable transportation and a valid driver's license
