The Mental Health Association is seeking a Case Manager to provide trauma-informed, person-centered support to individuals in substance use and/or mental health recovery who have histories of homelessness, assisting their transition from homelessness into studio apartments within a congregate supportive housing site.
Once housed, you will continue to provide ongoing support to help participants stabilize income and access services.
