Position Overview

The Resident Assessment Nurse (RAN) is responsible for conducting the Resident Assessment Instrument (RAI) process to identify residents’ medical, psychosocial, emotional and physical needs, and to develop care plans that support the highest practicable level of function. The RAN is an integral part of the facility team, attends standing meetings, contributes to interdisciplinary team (IDT) discussions, and meets the building’s expectations on conduct and professionalism.

Responsibilities and Duties
  • Ensure physical presence in the building and direct contact with residents, families, and the interdisciplinary team.
  • Conduct required resident interviews (BIMS, PHQ‑9, customary routines, pain assessment).
  • Perform Section GG functional observations and Section K nutritional assessments.
  • Gather, review, and synthesize supporting documentation across nursing, therapy, social services, and dietary.
  • Develop and maintain Care Area Assessments (CAAs), comprehensive care plans, and collaborate with the nursing team to keep resident care plans current.
  • Participate in daily clinical huddles, IDT meetings, care conferences, risk meetings and family meetings when necessary.
  • Coordinate with floor nursing on documentation needs that support accurate assessments.
  • Hand off completed clinical documentation to the central Resident Assessment Coordinator for technical coding and transmission.
Qualifications, Skills and Requirements
  • Experienced LPN with a minimum one‑year nursing experience in a skilled nursing/long‑term care facility;
    Registered Nurse (strongly preferred).
  • Resident Assessment Coordinator (RAC) certification preferred.
  • Excellent verbal and written communication skills with residents, families, all levels of staff, and external customers.
  • Proficient computer skills, including the ability to use Point Click Care and conduct online submissions.
  • Ability to work harmoniously with and supervise other personnel.
  • High degree of personal accountability, responsibility, and the ability to make independent decisions when circumstances warrant.
  • Ability to plan, organize, develop, implement, and interpret programs, goals, objectives, policies, and procedures necessary for providing comprehensive activities to residents.
