  • Create, plan, and organize a team as defined by the family
  • Develop, lead, and facilitate team meetings for the young person and their identified team
  • Provide care coordination within the family’s home, community, or preferred location
  • Partner with young people, families, natural supports, and other service providers to identify care coordination needs and services
  • Ensure the best outcomes for young people and families
  • Ensure youth and families have voice and choice in coordinated care and services
  • Maintain required documentation to access and maintain OhioRISE benefits
  • Meet quality and fidelity requirements defined by ISBH OhioRISE standards
  • Acquire and maintain knowledge of community resources
  • Attend mandatory in-person and virtual trainings and supervisions, requiring travel
  • Complete assigned trainings within identified timelines and maintain required certifications
  • Follow ISBH policies and procedures regarding confidentiality, professionalism, and incident reporting
  • Utilize databases and Electronic Health Record systems for accurate client documentation
  • Maintain compliance with HIPAA, mandated reporting requirements, and professional ethical guidelines
  • Travel within assigned service area
  • Perform other duties as assigned
Requirements
  • Experience providing services and/or support to children and families connected to behavioral health, child welfare, developmental disabilities, juvenile justice, or a related public sector human services or behavioral healthcare field is required: three years with a high school diploma or equivalent; or two years with an associate degree or bachelor's degree; or one year with a master's degree or higher
  • Knowledge and experience in Hi-Fidelity Wraparound preferred (Certification provided at time of employment)
  • Two years of experience in a coordinated supportive services or care coordination role that required high efficiency, organization, and attention to detail is preferred
  • Experience working with people with autism spectrum disorders and developmental disabilities is preferred
  • Experience in one or more of the following areas is required: family systems, community systems and resources, case management, child and family counseling or therapy, child protection, or child development
  • Qualifications and experience needed to work with children and families experiencing serious emotional disturbance (SED), trauma, co-occurring behavioral health disorders and engaged with one or more child serving systems
  • Effective verbal/written communication required
  • Ability to use a personal vehicle or company vehicle to provide services in homes, schools, and community settings required
  • Ability to maintain confidential information required
  • Ability to adhere to all safety, rules, regulations and requirements required
  • Ability to bill Medicaid without restrictions required
  • Valid driver's license and ability to operate a motor vehicle required
  • Appropriate level of auto insurance coverage required
  • Ability to manage deadlines within a fast-paced, high-volume environment required
  • Ability to operate in an Internet-based, automated office environment required
  • Ability to operate, receive and conduct on-camera video calls and/or meetings in an automated office environment required
  • Ability to maintain a minimum of 100mbps high-speed internet connection required
  • Prolonged periods of sitting at a desk and working on a computer required
  • Must be able to lift up to 15 pounds at times
