Overview

RN Case Manager - Full Time, Days - Care Management

RN Case Manager – Care Management role focused on patient advocacy, holistic assessment, care planning, and coordinating care across the continuum. The RN Case Manager ensures compliance with state and federal regulations and CMS Conditions of Participation. The role involves coordinating care recommendations from the multidisciplinary team to develop individualized plans that promote optimal health, access to services, and appropriate utilization of resources.

You’ll Do

The Registered Nurse (RN) Case Manager works collaboratively with multidisciplinary teams across the continuum of care to support, guide and coordinate care for patients, families and caregivers. Core values include patient advocacy, holistic assessment, care planning, and empowering patients, families, and caregivers to self-manage whenever possible. The RN Case Manager ensures compliance with state and federal regulations and CMS Conditions of Participation.

In the acute care setting, coordinates care recommendations to develop individualized plans with the goal of achieving optimal health and appropriate utilization of resources.

Responsibilities include:

  • Assess patient needs while evaluating health risks
  • Develop a comprehensive discharge plan that meets the patient’s needs
  • Coordinate all appropriate disciplines in the patient care
  • Optimize utilization management of resources
  • Perform interventions to reduce hospital length of stay and decrease readmission rates
  • Ensure that all post-acute care services are arranged prior to discharge
Education
  • Bachelor's Degree in Nursing (required)
Required Certification
  • Current Ohio License as a Registered Nurse
Required Experience
  • Five (5) years recent clinical experience or 3 years of inpatient hospital experience
