Registered Nurse; RN

Job Overview

We are a post‑acute care provider committed to family‑style patient care. As a Registered Nurse (RN) you will provide hospice and end‑of‑life care, working autonomously and within an interdisciplinary team.

PRN:
Minimum 16 hours per month.

• Provide one‑on‑one patient care and manage overall care from admission to discharge.
• Evaluate, individualize and update the patient plan of care to meet evolving needs.
• Administer medications and update all physician orders in the patient chart.
• Monitor patient status and determine if care level meets patient needs.
• Manage multidisciplinary care and promote continuity during admissions, transfers and discharges.
• Act as family educator and advocate, counseling patients and families about disease progression and care plans.
• Complete all clinical documentation following agency protocol and Medicare/Federal guidelines.
• Supervise total care of patient provided by nurse aides and LPNs and attend in‑service training and agency meetings.

• Registered Nurse with current license in the state of employment.
• Minimum one‑year experience as an RN in an acute care setting (preferred).
• Hospice experience desired.
• Valid driver’s license and insured auto insurance, and capacity to meet all physical demands required.
