• Active and unrestricted Registered Nurse (RN) license in the state of Ohio required
• Previous Home Health, Home Care, Skilled Nursing, or related clinical experience preferred
• Current BLS/CPR certification required
• Strong clinical assessment and documentation skills
• Ability to work independently in a home-based care environment
• Excellent communication, organization, and time-management skills
• Valid driver's license, reliable transportation, and current auto insurance required
• Ability to travel locally to patient homes throughout the assigned service area
• Ability to pass background screening and onboarding requirements

• Provide skilled nursing care to patients in their homes
• Complete comprehensive nursing assessments and monitor changes in patient condition
• Develop and follow individualized plans of care
• Administer medications and treatments as ordered
• Provide wound care and other skilled nursing services as appropriate
• Educate patients and caregivers regarding medications, diagnoses, treatments, and ongoing care needs
• Communicate changes in patient condition to physicians and other members of the healthcare team
• Coordinate care with physicians, therapists, caregivers, and other healthcare professionals
• Maintain accurate, complete, and timely clinical documentation
• Follow agency policies, clinical standards, and applicable regulatory requirements
• Promote patient safety, independence, and quality of life in the home environment

Job Description Wise Home Care is seeking an experienced Registered Nurse (RN) Home Care Field Nurse for Part-Time and Full-Time opportunities in Chillicothe, OH and the surrounding service area. The RN Field Nurse provides skilled nursing care directly in patients' homes, helping individuals safely remain in their home environment while receiving the clinical care and support they need. This position requires a nurse who is comfortable working independently in the field while remaining connected with physicians, caregivers, and the interdisciplinary care team.

Responsibilities include patient assessments, skilled nursing visits, medication and treatment management, patient and caregiver education, care coordination, and timely clinical documentation.
