- Active LPN license in Ohio
- Minimum 1 year of nursing experience required
- Home health experience preferred
- Current BLS certification required
- Ability to work independently and manage patient care in a home setting
- Strong communication and interpersonal skills
- Valid driver’s license and reliable transportation required
- Ability to pass background screening requirements
- Provide skilled nursing care to patients in their homes
- Perform wound care, medication administration, and vital sign monitoring
- Conduct patient assessments and monitor changes in condition
- Develop and follow individualized care plans in collaboration with the care team
- Communicate effectively with patients, families, and healthcare providers
- Maintain accurate and timely clinical documentation
