Administrative Registered Nurse RN

Administrative nurse needed for multi-faceted role that includes MDS/Care Coordinator/Infection Prevention/Wound Nurse

Join our compassionate and dedicated team at Livingston Manor Care Center, a unique and small skilled nursing facility in Chillicothe, MO, as a Minimum Data Set (MDS) Registered Nurse. We are seeking a talented and experienced MDS RN to help us provide exceptional care to our residents. As a key member of our care team, you will be responsible for conducting comprehensive assessments to develop individualized care plans, ensuring our residents receive the highest level of personalized attention and care.

• Conduct thorough assessments of residents, including MDS assessments, to identify care needs and develop individualized care plans
• Collaborate with interdisciplinary team members to ensure effective care delivery and coordination
• Provide education and training to residents, families, and staff on disease management, treatment options, and prevention strategies
• Monitor and evaluate resident outcomes, adjusting care plans as necessary to ensure optimal health and well-being
• Maintain accurate and timely records of resident assessments, care plans, and outcomes
• Participate in quality improvement initiatives to enhance the overall quality of care and services
• Round weekly with the facility wound physician
• Maintain the infection prevention program at the facility

• Current Registered Nurse (RN) licensure in the State of Missouri, or current multistate license
• Experience as a MDS RN, with a minimum of 0 years in a similar role
• Strong assessment and documentation skills
• Effective communication and interpersonal skills
• Ability to work collaboratively as part of a multidisciplinary team
• Strong problem-solving and critical thinking skills

This position requires working every other weekend as RN coverage and alternating call rotation with the DON
