Social Worker

Duties

The social worker will develop an assessment of the Veteran, family, economic, community, and psychosocial support systems. The assessments must reflect a high level of skill in gathering, analyzing, interpreting, and documenting data that is pertinent and directly related to patient health care planning, treatment, education and case management.

The social worker completes assigned sections of the Minimum Data Set on each Veteran admitted to the Community Living Center. The outcomes of these assessments are incorporated into the Veteran's plan of care. The social worker is responsible for facilitating the computerized RAI/MDS list and staying current with the assessments as assigned by the MDS Coordinator/designee.

The incumbent is responsible for contributing to the treatment plan, and setting achievable treatment goals with the Veteran/family in collaboration with the CLC interdisciplinary treatment team members. The social worker will include psychosocial problems in the interdisciplinary treatment plan and will attend weekly interdisciplinary care planning meetings.

Throughout the course of treatment on the CLC, the social worker is the subject matter expert on VA and/or community resources. The social worker will collaborate with other service providers in reassessing the Veteran's needs for non-institutional, institutional services/programs and entitlements.

The social worker is responsible for developing a resource file of VA and community social service programs and will refer the Veteran to needed services.

Social worker will act as an advocate when it serves the best interest of the Veteran/family. When appropriate and feasible, social worker will educate and encourage the Veteran/family to advocate for themselves, thus fostering a sense of independence and empowerment.

Incumbent is experienced in making rapid assessments and developing crisis management plans. Social worker has access to multiple resource directories in addition to previously developed resources to meet the demands of a crisis. In the event of a medical or psychiatric emergency, staff will follow the protocol of the Community Living Center.

Social worker provide education related to VA and community resources, entitlements, Advance Directives/Living Will and will refer Veterans/families to the appropriate interdisciplinary team member for identified health education needs.

This position may requires the ability to accompany and transport Veterans to appointments and other community resources as needed, whether medical, housing, or other treatment plan goal appointments through the use of a government vehicle.

Work Schedule:

Monday through Friday, 7:30 am to 4:00 pm.

Pay:
Competitive salary and regular salary increases.

Paid Time Off: 37-50 days of annual paid time offer per year (13-26 days of annual leave, 13 days of sick leave, 11 paid Federal holidays per year) Selected applicants may qualify for credit toward annual leave accrual, based on prior [work experience] or military service experience.

Parental Leave:
After 12 months of employment, up to 12 weeks of paid parental leave in connection with the birth, adoption, or foster care placement of a child.

Child Care Subsidy:
After 60 days of employment, full time employees with a total family income below $144,000 may be eligible for a childcare subsidy up to 25% of total eligible childcare costs for eligible children up to the monthly maximum of $416.66.

Retirement:
Traditional federal pension (5 years vesting) and federal 401K with up to 5% in contributions by VA.

Insurance:
Federal health/vision/dental/term life/long-term care (many federal insurance programs can be carried into retirement).

Telework:
Ad-hoc only telework is authorized as determined by the needs of the agency.
