Licensed Vocational Nurse (LVN) - NOC Shift Trellis Chino

Shift: 11:00 PM - 7:00 AM (NOC)

Schedule:

4 Days On / 2 Days Off Rotation Pay Range: $29.00 - $33.00 per hour, DOE

Join the Team at Trellis Chino Trellis Chino is a 59-bed short-term skilled nursing facility conveniently located near Chino Valley Medical Center. Our facility features private suites and a patient-centered approach focused on providing exceptional clinical care and outstanding resident experiences.

We are currently seeking compassionate and dedicated Licensed Vocational Nurses (LVNs) to join our NOC shift team.

• Administer medications and treatments according to physician orders
• Accurately document patient care using electronic medical records
• Monitor and assess resident conditions, reporting changes as appropriate
• Provide care for residents with acute and chronic healthcare needs
• Supervise and support Certified Nursing Assistants (CNAs)
• Communicate effectively with residents, families, physicians, and interdisciplinary team members
• Utilize critical thinking skills to make sound clinical decisions
• Ensure compliance with all facility policies, procedures, and regulatory requirements

• Current California LVN license in good standing
• Strong clinical assessment and documentation skills
• Excellent communication and interpersonal abilities
• Ability to work independently and collaboratively within a team environment
• Compassionate and resident-focused approach to care
