Overview

Schedule:

PRN — as needed coverage when clinic is open (M-F). No set hours or consistent days. Typical shift: 8:00 am – 4:30 pm. Ideal candidate is open to supporting both Eau Claire and Chippewa Falls locations.

• Provide professional nursing care to patients in accordance with the Nurse Practice Act, state Administrative Code, and hospital policies.
• Consult with the health care team as the patient’s condition and treatment goals warrant; collaborate with Patient Accounts to provide patient information for compliance with federal and state statutes.
• Complete initial assessments and document findings.
• Collect, record, and analyze data for admission and assessment per hospital policy, including patient strengths, cultural/spiritual/ethnic factors, discharge needs, medical/physical status, medication history and adherence, and age-specific needs.
• Involve patient support systems (family, friends) in assessment and documentation; observe interactions and obtain assessment data when appropriate.
• Act as a patient advocate, explain patient rights, obtain signatures on consents, and support patient orientation while upholding privacy and confidentiality.
• Initiate and update the treatment plan; participate in planning and modifying care plans; evaluate multidisciplinary assessments and present nursing care components in staffing.
• Develop and interpret the plan of care with the patient/family and update goals as indicated; write clear, obtainable treatment goals for each problem and review as goals are achieved or updated.
• Document ongoing nursing information from interviews, observations, assessments, and diagnostics; confer with health care professionals as appropriate.
• Review current lab data, follow up with doctors, and assess and implement fall precautions and safety measures as indicated.
• Identify risks such as self-harm, suicidality, or assaultive behavior; develop age-appropriate interventions and adapt the plan of care accordingly.
• Provide safe medication administration: verify right patient, medication, dose, time, and route; document responses and monitor for adverse effects and interactions; educate patients about medications; transcribe and implement physician orders; stay current on pharmacologic knowledge.
• Identify health education needs before discharge and deliver age-appropriate teaching; document in the patient record and treatment plan; organize care activities per patient priorities, unit needs, and time constraints.
• Ensure admissions, transfers, and related activities comply with EMTALA and regulatory requirements when applicable; adhere to nursing and hospital standards of practice.
• Maintain safety and infection control standards to protect patients, staff, and visitors; participate in continuing education and in-service programs; report information to unit staff; attend required in-services and participate in Performance Improvement programs.
• Assist in development and implementation of unit standards of care (e.g., safety levels, Kardex) and standard care plans; identify unit educational equipment and supply needs; serve on unit-based committees or projects as appropriate.
• Provide leadership as charge nurse when assigned: coordinate unit activities, lead in crisis situations, facilitate admissions, communicate unit status, manage the milieu, and plan patient care assignments to ensure continuity and quality of care; supervise and delegate tasks to LPN/psychiatric technicians; maintain primary caseload with direction for patient care; support staff and patient safety.
• Participate in projects, tasks, and continuing education to improve professional skills and unit systems; develop goals for professional growth and pursue opportunities to advance unit and department objectives.
• Promote department goals and the hospital mission; communicate goals, demonstrate measurable achievement, and maintain policies; comply with external regulatory requirements and educate new staff on regulations as directed.
