Registered Nurse- CBOC PACT RN Care Manager

Job Title

Major duties include, but are not limited to:

• Collaborates with services internal and external to the VA to facilitate care transition to effectively meet the patients' needs.
• Provides patient & family health education with a focus on self-management, prevention, and wellness, based on the patient's goals.
• Triages and applies a collaborative team approach in identifying, analyzing, and resolving patient care problems.
• Demonstrates leadership by serving as an advocate for patients as they continue to enhance their professional growth, development & practice.
• Administration & Monitoring of Medications
• Facilitation of Homecare/Community referrals and recommendations to support ongoing veteran needs
• Works collaboratively with all levels of health care professional and support staff to facilitate positive patient care outcomes.
• Maintains prevention & management of disruptive behavior skills, can be trained upon hire
• Promotes evidence-based best practices and nurse sensitive indicators (including but not limited to Falls Prevention, Whole Health, PACT)

Pay:
Competitive salary, regular salary increases, potential for performance awards
