Role:
- Conduct psychosocial assessments and develop individualized service plans.
- Provide counseling and case management to clients across behavioral health, housing, and family services.
- Coordinate community resources to support client needs.
- Collaborate with an interdisciplinary team to ensure comprehensive care.
- Maintain accurate documentation of client interactions and service plans.
- Advocate for clients, promoting dignity, inclusion, and social justice in a supportive environment.
