Job Overview:
We are currently seeking a compassionate and skilled Home Health Licensed Practical Nurse - LPN to provide one-on-one care for a patient in a home setting in Christiansburg, VA. This position is ideal for an LPN who is comfortable with hands-on care and enjoys building meaningful connections with patients and families.

As a Home Health Licensed Practical Nurse - LPN, you will be providing care for a patient with a PEG tube, supporting both the patient and their family in the home environment.

Responsibilities of Home Health Licensed Practical Nurse - LPN:

* Provide direct patient care in a home setting

* Administer and monitor PEG tube feedings

* Perform routine assessments and monitor patient condition

* Assist with medication administration as prescribed

* Maintain accurate documentation of care and patient progress

* Support the patient and family in creating a safe, clean, and comfortable environment

* Communicate effectively with family members and care team

Qualifications of Home Health Licensed Practical Nurse - LPN:

* Active LPN license in the state of Virginia

* Experience with PEG tube care and feedings preferred

* Strong clinical and critical thinking skills

* Compassionate, reliable, and patient-focused

* Ability to work independently in a home setting
