Nurse Practitioner or Physician Assistant - PRN

Responsibilities

During a visit that can last up to one hour, Matrix providers review and observe a member’s current health, medical history, medication adherence, social environment and other risks. This provides unmatched insight into a member’s overall health and well‑being that can be difficult to capture during routine office visits.

• Conduct adult and geriatric assessments to include medical history, diagnosis and treatment, health education, physician referrals, case management referrals, follow‑up and clear documentation according to Matrix guidelines and protocols.
• Administer and order point‑of‑care and lab‑based testing as needed.
• Work with physicians, case managers, social workers, family members, key caregivers, and ancillary medical personnel as appropriate.
• Collaborate with Primary Care Physician on patient education and follow‑up as necessary.
• Provide services at the member’s place of residence which can be in homes or Skilled Nursing Facilities.

• Master’s degree or commensurate experience and satisfactory completion of NP licensure.
• Current NP licensure in state of practice to include prescription authority or ability to obtain prescriptive authority as needed.
• AANP, AACN, or ANCC board certification in Adult, Adult Gerontology Primary Care, Adult Gerontology Acute Care, Family, Emergency, or Acute Care specialty.
• Current BLS, ACLS or CPR certification.
• One year of NP experience preferred; new graduates encouraged to apply.
• Valid state driver’s license for the state of residence.
• Automobile insurance coverage meeting or able to obtain state requirements (bodily injury: $100,000 per person / $300,000 per incident; property damage: $50,000; collision / comprehensive deductibles: $1,000 or less).
• Strong computer skills and familiarity with employee health/medical record software.
• Excellent verbal and written communication skills.

• Flexibility:
  Visits are scheduled based on your availability, from Sunday to Saturday, 8 am to 7 pm, with options for day, evening, or weekend shifts.
• Competitive Pay:
  Earn income based on the number of assessments you complete; potential earnings range from $1,800 to $11,500 per month, depending on assessment rate and state.
