- 1+ year experience in hospice
- Valid driver's license
- Good driving record and reliable transportation
Roles & Responsibilities :
- Appropriately handle calls either by phone or with home or facility visits when necessary or requested.
- Provide education and emotional support for patients and families.
- Electronically communicate all activity upon completion of on-call shifts.
- Document pertinent calls and visits electronically.
