Requirements:

• Current state license as a Registered Nurse
• Homecare Homebase Software Experience
• OASIS Experience
• Proof of current CPR certification
• Current driver's license
• Reliable transportation and insurance
• 1-2 years experience as a Registered Nurse in a clinical care setting
• 2+ years home health experience
• CHPN certification (preferred)

Roles & Responsibilities:

• Assess patients in home health and hospice settings to identify their physical, psychosocial, and environmental needs.
• Develop, implement, and document the Plan of Care (POC) to ensure quality and continuity of care.
• Educate patients and families about care plans, medications, and safety measures.
• Supervise and provide clinical direction to home health aides to ensure quality service delivery.
• Monitor compliance with third-party payer requirements and assess pain management needs.
• Provide effective communication and serve as a liaison among patients, families, and healthcare team members.
