We are currently seeking a compassionate and skilled Home Health Licensed Practical Nurse - LPN to provide one-on-one care for a patient in a home setting in Christiansburg, VA. This position is ideal for an LPN who is comfortable with hands-on care and enjoys building meaningful connections with patients and families. As a Home Health Licensed Practical Nurse - LPN, you will be providing care for a patient with a PEG tube, supporting both the patient and their family in the home environment.
Responsibilities- Provide direct patient care in a home setting
- Administer and monitor PEG tube feedings
- Perform routine assessments and monitor patient condition
- Assist with medication administration as prescribed
- Maintain accurate documentation of care and patient progress
- Support the patient and family in creating a safe, clean, and comfortable environment
- Communicate effectively with family members and care team
- Active LPN license in the state of Virginia
- Experience with PEG tube care and feedings preferred
- Strong clinical and critical thinking skills
